Alien Mind. If a creature tries to read a servant of the eye’s thoughts or deals psychic damage to it, that creature must succeed on a INT saving throw or be stunned for 1 minute. The stunned creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.   Amorphous. The servant of the eye can move through a space as narrow as 1 inch wide without squeezing.   Hold Breath. The servant of the eye can hold its breath for 1 hour.   Illumination. The servant of the eye sheds dim light in a 20-foot radius.   Legendary Resistance (3/day). The servant of the eye can turn a failed save into a success.   Magic Resistance. The servant of the eye has advantage on saving throws against spells and other magical effects.   Radiant Mirror. If the servant of the eye takes radiant damage, each creature it has line of sight takes the same damage as well split evenly between the targets. This reflected damage is unaffected by its resistances.   Reactive Tentacles. For each tentacle the servant of the eye has beyond one, it gets an extra reaction that can be used only for opportunity attacks.   Servant of the Eye. The servant of the eye can be controlled with a tentacle rod.   Spider Climb. The servant of the eye can climb difficult surfaces without a check.   Tentacled Horror. The servant of the eye has ten tentacles. Whenever the servant of the eye takes 25 or more slashing damage in a single turn, that targets a tentacle, one of its tentacles is lopped off. Servants regenerate all lost tentacles within 1d12+11 hours.

Actions

Multiattack. The servant of the eye makes as many attacks as it has tentacles (usually 10). It will typically try and strangle as many targets as possible as opposed to using its tentacles against one target to overwhelm it.   Stranglehold. Melee Weapon Attack: 1d20+14 to hit, reach 20 ft, one target. Hit: 3d6+9 bludgeoning damage, and target is grappled and restrained (escape DC 17). The servant of the eye can grapple upto ten creatures this way. A grappled creature automatically takes 3d6+9 bludgeoning damage at the start of its turn. If this attack is a critical hit or two tentacles hit the same target, the target also can’t breathe or speak until the grapple ends as the servant of the eye strangles its target. A target being strangled suffocates to death after a number of rounds equal to its CON modifier (minimum of 1 round).  

BONUS ACTIONS
Maddening Presence. The servant of the eye targets one creature it can see within 30 feet of it. If the target can see or hear the servant of the eye, the target must make a WIS saving throw. On a failed saving throw, the target becomes paralyzed until the end of its next turn. If a creature’s saving throw is successful, the creature is immune to the servant of the eye’s Maddening Presence for the next 24 hours.

Reactions

Scurry. The first time the servant of the eye is bloodied, the servant of the eye can take the Disengage action and moves double its speed.

Legendary Actions

The servant of the eye can take 3 Legendary Actions, choosing from the options below. Only one legendary action option can be used at a time, and only at the end of another creature's turn. The servant of the eye regains spent legendary actions at the start of its turn.

  • Stranglehold. The servant of the eye uses Stranglehold.
  • Tentacle Whip. The servant of the eye makes a stanglehold attack, but instead of grappling a target it must succeed on a INT saving throw or be stunned until the end of the targets next turn.
  • Mind-Weakening Ray (Recharge 5-6). The target must succeed on a INT saving throw or take 36 ( 6d6 ) psychic damage and be unable to cast spells or activate magic items for 1 minute. The target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.

Servants of the Eye are horrors from beyond the regular boundaries of the multiverse, having come to the Prime Material Plane with their master, the Elder Elemental Eye (also known as the Elder Elemental God). They live only to serve the Eye, and without its active presence to guide them, descend into either listlessness (20% chance) or homicidal savagery (60%). Some few (20%) will have received a simple and obeyable order prior to the final overthrow of the Eye, and will still be following that instruction millennia later. Those carrying a lesser or greater tentacle rod are able to control Lesser Servants of the Eye.   Servants of the Eye are vaguely squid-like creatures with ten hairy tentacles. Their slimy flesh is black and red. They move remarkably swiftly on land for all their bulk.
