Features
Fractured Strike

When you hit a creature with this weapon, it fractures the mind as well as the body. The target must succeed on a DC 16 Wisdom saving throw or take an additional 2d10 psychic damage and gain 1 level of Madness (explained below).

Burst of Madness (Recharge 6 rounds)

As an action, you can unleash a wave of Verathis’s chaotic power. All creatures within a 20-foot radius must make a DC 16 Wisdom saving throw. On a failed save, creatures take 4d10 psychic damage and are frightened of you until the end of their next turn. On a success, they take half damage and are not frightened. Creatures frightened in this way must use their movement to flee from you until they can no longer see you.

Madness Infusion

When you throw this spear, it leaves behind a trail of maddening energy. The thrown weapon deals an additional 2d6 psychic damage on a hit. Upon impact, all creatures within a 10-foot radius must succeed on a DC 16 Wisdom saving throw or gain disadvantage on saving throws against being frightened or charmed for 1 minute.

Madness Mechanic

Whenever a creature gains a level of Madness from the spear’s effects, the following penalties accumulate:
• 1 Level of Madness: The creature hears whispers and experiences visual distortions, imposing disadvantage on Wisdom (Perception) checks.
• 2 Levels of Madness: The creature has disadvantage on all attack rolls and saving throws involving Wisdom, Charisma, and Intelligence.
• 3 Levels of Madness: The creature becomes confused (as per the Confusion spell) for 1 minute, with no save.
• 4 Levels of Madness: The creature’s mind fully shatters. It is incapacitated for 1 minute and takes 3d8 psychic damage at the start of each of its turns until it succeeds on a DC 16 Wisdom saving throw to reduce its Madness by 1 level.

Madness levels are reset after a long rest or through a Greater Restoration spell.

Cursed by Verathis

The spear is imbued with the insanity of its creator, and its wielder is not immune to its effects. Upon attuning to this weapon, you gain the following flaw:
“Reality is fractured, and the shards reveal truths beyond comprehension.”

After each long rest, you must succeed on a DC 16 Wisdom saving throw or suffer 1 level of Madness for the day. Madness caused by this curse cannot be removed by mundane means and persists until the attunement is ended.
